CHART Online

CHART Online works in conjunction with CHART. The CHART tool permits easy and secure uploading of IM&T DES summary data to CHART Online. CHART Online then allows GP (general practice) practices and PCT staff to compare the data against national and local peer sets. CHART Online has a variety of comparative viewing options available:

  • overall practice position.

  • progress of data quality improvement over time.

  • position against national or local peers.

The results are displayed as interactive graphs or tables. By clicking on specific areas in the displayed data it is possible to drill down from SHA level to the individual practices. To accompany the data there is also help information tailored specifically to the graph or table being displayed.

To access CHART Online and view the analyses, you must first register on the Profile Centre. The link can be found in the column on the right. Further instructions can be found on the Profile Centre once registered. After registration you must then upgrade your account to Level 2.

Please note that only practice staff and PRIMIS+ facilitators can upgrade to Level 2.

Currently CHART Online contains data on:

  • Data quality

  • GRASP-AF.

  • Heroin misuse

  • IM&T DES

  • Myocardial Infarction and Acute Coronary Syndrome
