PRIMIS+ provides a range of data extraction queries delivered by the PRIMIS+ CHART software. These tools and query libraries are available free to Primary Care Organisations (PCO) who have undergone PRIMIS+ training.

The PRIMIS+ Information Team provides sets of queries to help general practices assess the quality, completeness and consistency of their data recording. This analysis using the CHART and CHART Online software helps practices to identify areas for improvement and develop action plans:

• CHART
  
  CHART (Care and Health Analysis in Real Time) works in conjunction with CHART Online. CHART is a software tool from PRIMIS+ that helps GPs improve patient care by analysing the data held on their clinical computer systems. The software is available free to NHS users in England and can be accessed through registering on the PRIMIS+ Profile Centre

• CHART Online
  
  CHART Online works in conjunction with CHART. The CHART tool permits easy and secure uploading of IM&T DES summary data to CHART Online. CHART Online then allows GP practices and PCT staff to compare the data against national and local peer sets
